The Crown of the Sepsis Whore
Donut's first legendary prize, a cat-sized tiara of smoky gems she puts on before anyone can read the warning: whoever wears it is placed permanently in the Blood Sultanate's royal line and must slay the Sultan and every other royal before leaving the ninth floor; fleeting, so that another crown appears elsewhere if it comes off. 'It's purple. Purple is my color.'

In hindsight, after book 5
The tiara Donut puts on in the tutorial before anyone can read the warning is the mechanism of book 5's ending. Its description says the wearer must slay the Sultan and every royal before leaving the ninth floor, and that it is a Fleeting item; Mordecai explains that if it comes off another appears elsewhere, one more person to kill to get off that floor. Donut loses hers at the end of the third floor, and the book-5 line that recites the description from memory is the setup: the crown that regenerated was in Eva's keeping, and in the last chapter it goes onto Katia's. 'Only one of them, Donut or Katia, would be allowed to take the stairs from the ninth to the tenth floor.'
As of book 6. Donut's first legendary prize in book 1, lost on the third floor and regrown into Eva's keeping, put on Katia as Eva died. Book 6 opens on it: a private message, a plan that starts with a goddess, 'This is just like with Ferdinand', 'It's either her or Donut. […] Some breaks aren't clean.', and an orchid won with Astrid's death that nobody wants eaten. Only one of them takes the stairs from the ninth floor.

Donut's first legendary prize and the first thing she equips: a tiara of smoky purple gems whose description, read too late, places her in the Blood Sultanate's royal line and requires her to kill the Sultan and every royal before leaving the ninth floor (B1 ch. 7). Fleeting — take it off and another crown appears elsewhere. 'That crown is on her head, not yours.'
Through book 5
Donut lost the tiara at the end of the third floor, but the requirement stayed: she cannot leave the ninth floor until every royal of the Blood Sultanate is dead (B5 ch. 12). The regenerated crown was 'possibly awarded to someone else' (B5 ch. 42); a decoy in a crown (B5 ch. 35); Donut flagged as Blood Sultanate royalty on the Faction Wars roster (B5 ch. 77); and in the last chapter Eva, dying, pulls it from the back of her breastplate and puts it on Katia, so that only one of Donut and Katia may take the stairs from the ninth floor (B5 ch. 78).

Katia's private message: the Crown is on her head, and there is a way out, beginning with resurrecting Yemaya (ch. 15); Eileithyia promises in a deity message to clear the issue, pointing to what a goddess in her own temple can do (ch. 17); the crown on Katia's head, on Carl's mind (ch. 18); Donut sees the purple tiara and goes rigid — 'This is just like with Ferdinand' — and there are two ways out, the pregnancy boon and the Celestial Attendant boon (ch. 19); Donut's replacement tiara of Mana Genita (ch. 22); Mordecai, privately: 'It’s either her or Donut', and in his view Katia self-sabotaged her own solution (ch. 30); the orchid won and not wanted (ch. 72).
